Pathways to Hospice

When people think of hospice, they often think of it as a place, and while it was once considered a place, the term has come to mean much more. At its heart, hospice represents a philosophy of holistic, compassionate care provided to anyone facing an end-of-life illness.

It was with this philosophy in mind that we developed our Pathways Program.

Pre-Hospice Care

When deciding to enter a hospice program is not yet acceptable to a patient facing a life-limiting illness, Hospice Associates will voluntarily coordinate a pre-hospice palliative management program supervised by our medical directors. Until such a time when transitioning to hospice becomes desirable.

Home-Based Care

Most patients facing a life-limiting illness prefer to receive hospice services in the privacy and comfort of familiar surroundings at home. Hospice care is a service that transcends locations and can be offered at any site that meets the necessary criteria set by the Center for Medicare and Medicaid Services, including a private home setting.

Intravenous Therapy

When a patient’s pain control can no longer be managed effectively with oral and topical medications, Hospice Associates can provide intravenous medications using micro-pumps and other devices programmed to deliver adjustable doses to achieve comfort goals.

Our staff will provide the equipment, training, and assistance to reliable caregivers to help provide hospital-quality pain management at home.

Inpatient Care

When hospice care at home is no longer an option due to the severity and acuity of symptoms, or when hospice care is unable to provide the level of care needed, we offer and coordinate with insurance carriers to transition to our inpatient hospice programs.
