Electronic Communications Terms & Conditions

Care Associates (“we,” “us,” or “our”) offers electronic communication services, including SMS text messages and email (“Electronic Communications”), to provide timely, relevant information to patients, caregivers, and partners regarding healthcare services, reminders, care coordination, and related updates.

By opting in to receive these communications, you agree to these Terms and Conditions.

1. Message Types and Frequency

Electronic Communications may include, but are not limited to:

  • Appointment confirmations and reminders
  • Care plan updates and coordination messages
  • Enrollment confirmations
  • Program eligibility notifications
  • General service-related alerts

Message frequency will vary depending on your care engagement and preferences but will not exceed what is reasonably necessary for your care and coordination.

2. Opting In

You may opt in to receive communications by:

  • Submitting a web form, intake form, or consent form with explicit approval
  • Checking an authorization box during service onboarding
  • Texting a keyword (e.g., JOIN) to our designated SMS number (for SMS only)

By opting in, you confirm that:

  • You are the authorized user of the phone number and/or email address provided
  • You consent to receive non-marketing, service-related communications

3. Opting Out

You may opt out of receiving SMS messages by replying: STOP, CANCEL, QUIT, UNSUBSCRIBE, or END

You may opt out of receiving emails by clicking the unsubscribe link provided at the bottom of each email.

Once you opt out, you will no longer receive communications through that channel. You may re-subscribe by providing consent again through our intake process or subscription form.

Note: Opting out may impact on our ability to coordinate your care efficiently.

  • SMS: We do not charge you for text messages; however, message and data rates may apply depending on your wireless provider and plan.
  • Email: No charges apply for email communications.

You are responsible for any carrier or data charges.

6. Privacy and Data Use

We respect your privacy and comply with HIPAA and applicable data protection laws. SMS and email messages may include protected health information (PHI) relevant to your care.

By using these services, you agree to our Privacy Policy and authorize us to send you messages as described. We will not sell, share, or rent your phone number, email address, or PHI for marketing purposes.

7. Security Disclaimer

While we use industry-standard safeguards, SMS and standard email are not fully secure forms of communication. Please avoid sending sensitive information (e.g., Social Security Numbers, financial data) via these channels.

For sensitive matters, we may provide a secure link to our patient portal.

If you prefer not to communicate by SMS or email, you may opt out at any time.

8. Service Availability and Changes

We may suspend, modify, or discontinue the SMS or email services at any time without notice. Your continued use constitutes acceptance of any changes to these Terms.
