Improving the dementia experience for patients and their caregivers
Care Associates has been selected by the Centers for Medicare & Medicaid Services (CMS) to participate in its GUIDE Model (Guiding an Improved Dementia Experience), a new, alternative case management, care coordination, and medical care model designed to support patients living with dementia and their caregivers.
What is the GUIDE Program?
The GUIDE Model focuses on comprehensive, coordinated care and aims to improve the quality of life for people with dementia, reduce strain on their unpaid caregivers, and enable them to remain in their homes and communities longer. It will achieve these goals by providing funding for personalized, comprehensive care coordination and management, caregiver education and support, and respite services.

GUIDE Program Benefits
Comprehensive Assessment
Eligible beneficiaries and their caregivers participate in an initial comprehensive assessment, followed by annual follow-up evaluations, conducted in a survey format. These assessments are designed to identify individual needs and create a tailored plan of care.
Referral & Support Coordination
A Care Navigator helps patients and caregivers connect with valuable community-based resources and services.
Care Plan
An individualized care plan is carefully designed to align with the goals, preferences, and needs of patients and their caregivers.
Caregiver Support
Caregivers can attend educational classes, while patients benefit from respite services, easing the burdens and responsibilities of caregiving.
24/7 Access
Patients and caregivers have round-the-clock access to a helpline for continuous support and assistance.
Ongoing Monitoring & Support
Care Associates offers long-term support to patients and their caregivers, enabling them to review and update their goals and needs at any time.
Medication Management
A clinician ensures medications are reviewed and reconciled as needed, while care navigators provide helpful tips for maintaining an accurate and safe medication schedule.
Care Coordination & Transition
Patients are promptly referred to specialists for additional health concerns, with care navigators coordinating the care process to ensure seamless support.

Am I eligible for the GUIDE Program?
You are eligible to receive services under the GUIDE Model if you meet the following criteria:
• You have previously been diagnosed with dementia, as confirmed by a Care Associates clinician reviewing your history and legacy medical records available, and you reside in inside the zip code-based service areas.
• Traditional Medicare Parts A and B constitute your primary health insurance AND are you have not enrolled in any Medicare Advantage, including Special Needs Plans, nor have enrolled in a PACE program.
• You are not enrolled in the Medicare hospice benefit, and;
• You are not eligible if you reside in a nursing home. However, assisted living and group home facilities are not classified as nursing homes, and you may qualify if you live in one of these settings, provided all other eligibility requirements are met.
• You consent to receive GUIDE Program services, including medical services, through Care Associates (participation in the program does not restrict your your ability to receive services from any other provider or supplier).
If I am eligible for GUIDE program, what can I expect?
If you think you are eligible because you met the criteria listed above, you should call our office at 504-457-2200 and ask to speak to one of our care navigators. The GUIDE program is a voluntary program requiring your prior authorization to be enrolled in it. Our navigators will ask for your contact information, verify you meet GUIDE criteria, and schedule a home or telehealth visit with one of ur providers to complete an initial, comprehensive assessment required by the program.
Can I still use Care Associates if I already have a primary care provider?
Yes, you can keep your primary care provider, or elect to use Care Associates as your new primary care provider. You will also continue to have access the other specialty physicians participating in your care.
How do I enroll in the GUIDE program?
You can voluntarily refer yourself for enrollment only through a participating provider like Care Associates. If you think you meet the criteria listed above, you can contact Care Associates to sponsor and assist you in completing the screening and enrollment process.
